Maryland ordering instructions

MDVIPAttention MDVIP Patients: We are already working with your physician, so the following process does not apply to you. Please continue to complete checkout and we will coordinate approval with your physician. What is MDVIP?

The State of Maryland requires that all genetic services and tests be ordered by a physician on behalf of residents. You may still place your order, but you will need to complete a few additional steps to get your physician's required approval before we can ship your test kit to you.

  1. Complete the checkout process using your Maryland address.
  2. Download and print this Navigenics Test Request Form (TRF) for Maryland orders.
  3. Fill out as much information on the form as you can. Please include your shipping information.
  4. Take the printed form to your physician and have them fill in the required fields to certify that they approve this test.
  5. Have your physician (or yourself) FAX the completed form back to Navigenics Member Services at (650) 585-5661.

We will then ship your Navigenics collection kit to you.
